We are excited to have your family join our practice! Please click here to fill out the form to transfer your child's records from your current practice to ours. Complete the form and send it to your child's current medical provider(s). We are so sorry to see your family go. We wish you the best and want to make the process as streamlined as possible.

• When you have chosen a new provider, download, complete, and provide us with a signed Medical Release Form. In the section heading "This information may be disclosed to and used by the following organization," write the new physician contact, the medical organization name and contact information.
• If you are requesting records for your own use, there will be a $15 charge plus 35 cents per page over 10 pages.
• Your request to have your medical records transferred will be completed by our Medical Records department within 10 business days via fax. A paper or digital copy of the records may also be requested.
